Job summary Through strong clinical leadership and effective management, the Parkinson's CNS will support the Neurology Service to promote and deliver nurse excellence for adults with Parkinson's. This role requires a high level of competence, nursing expertise and clinical decision-making skills. To be able to work autonomously in managing a defined caseload of adults with Parkinson's. The post holder will be involved in direct patient care for adults with Parkinson's post diagnosis through to the palliative stages of the illness, by providing expert advice, assessment and medication management to patients, their families/carers and professionals. You will be involved in the wider Neurology team meetings and actively participate in these regarding the care of adults with Parkinson's. Main duties of the job The jobholder will ensure that all patients are treated with compassion, dignity and respect placing value on the diversity of the local community and beyond. He/she will be a highly motivated individual who can work as part of a team but also practice autonomously managing a complex caseload of patients. You will work collaboratively with Consultant Neurologists and be responsible for running Nurse led clinics, arranging home visits as well as community clinics. You will be a source of expert knowledge related to idiopathic Parkinson's Disease and Parkinsonism for all health and social care professionals. To have local open and transparent communication channels that support the team to do the right thing for people we care for, to be bold when they have good ideas and to speak up when things go wrong to ensure learning and quality improvement. As a key member of a multi professional team, ensure appropriate referral, signposting and discussion with other Teams and Services to optimise patient care. Ensure patients are assessed, have care plans that are implemented and evaluated in accordance with the Trust's and NMC requirements.
