Main duties of the job
HWICB are looking to recruit a highly motivated, and enthusiastic expert in CHC in this dynamic and rewarding role. The role is designed to take the operational lead for delivering the strategic direction and delivery/performance of the busy Clinical Operational All-Age Continuing Healthcare service across Herefordshire and Worcestershire.
The post holder will work collaboratively with other Heads of CHC Service and Associate Director of Nursing for CHC, as part of an innovative Leadership team which is part of NHS Herefordshire and Worcestershire Integrated Care Board (HWICB).
Detailed job description and main responsibilities
As the Head of CHC Operational Lead the post holder will take direct managerial leadership for delivering a high quality, effective and efficient CHC service with a clear focus on person-centred care with consistency and quality in the application of the National Framework as well as service development, transformation, and financial accountability.
They will work with all stakeholders to ensure service planning and developments reflect identified local needs. The post holder will provide clinical leadership and management of the CHC Operational service which includes the CHC process from referral to verification, Fast Tracks, DTA and pathway 4 discharges.
The post holder will adhere to locally agreed quality, performance and financial targets and will be responsible for regularly reporting progress against the corporate and operational objectives.
The post holder will lead and manage a large and diverse team including Clinical Leads and Clinical Team leaders across the Assessment and Fast Track team.
The post will be responsible for actively working to support the achievement of our Efficiency programme whilst maintaining a high-quality continuing healthcare service.

Qualifications
* Current clinical professional registration Clinical Qualification (e.g. nursing or therapy).
* Evidence of ongoing continued professional development
Skills
* Highly developed skills in planning, managing and organising complex multi-agency work to achieve good outcomes
* Working knowledge of the NHS Continuing Healthcare National Framework, associated directives, regulations and guidance
* Relevant knowledge and application of change management
* Evidence of analytical skills, advanced report writing and presentation skills.
* Ability to plan, organise and coordinate a wide range of activities within short timescales and often when there are conflicting priorities
* Ability to identify risks, anticipate issues and create solutions and to resolve problems in relation to project or service delivery.
Experience
* Specialist knowledge and experience of key national policies relating to ‘NHS Continuing Healthcare and NHS Funded Nursing Care’ including complex areas of continuing healthcare such as PUPOCs, PHB, S117 funding, appeals and complaints management.
* Minimum of 2 years’ experience of working at a senior level in CHC services.
* Experience of building and sustaining effective and productive working relationships with a wide range of people at all levels and across a wide variety of agencies.
* Experience of managing allocated resources and budgets including setting, monitoring and forecasting
* Experience of developmental work with local authority and other non-NHS partners.
