Job summary We are looking for a Complex Discharge Tracker to join our Home First, Discharge to Assess Service based in Burton on Trent, Staffordshire. The Home First service is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs. The service seeks to offer support to people ranging from those that need minimal support for a time limited period to rebuild confidence, learn new skills and relearn skills through to people at the end of their life requiring skilled health & social care interventions. You will be enthusiastic and passionate individual and be motivated, flexible and an effective team player, to prioritise and implement effective negotiation and communication skills. You will be responsible for the scheduling and allocating care calls on the Complex Discharge Tracker System. They will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales. They will ensure that the Complex Discharge Tracker system is validated and updated within each locality teams. The service operates 7am to 10pm 365 days per year and you will work on a shift pattern basis to cover those hours. Main duties of the job As a Complex Discharge Tracker, you will review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale. Dependent on capacity and demand of the Home First Teams ensure that any unmet referrals are immediately escalated for authorisation by the Team Leader for the individualised plan of care to be sourced from external partners. As part of this role, you will be expected to: Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day. Ensure that every change to a patient's package of care (reductions, increases, cancellations, closures etc) is accurately input into CDT, and that every change has a corresponding written explanation recorded in CDT to document the reasons why the change was made to provide an audit trail. Realign the allocated rosters and ensure that the allocated care calls are within the correct geographical areas to enable the team to demonstrate efficiency. Actively work with the Shift Leads to find appropriate staff cover to cover unplanned sickness or emergency cover as required.
