Job summary
Work in collaboration with Bevan colleagues todeliver a coordinated and high-quality service that proactively encourages serviceusers to access and engage with community support. To work as part of theHealth Inclusion Outreach Team through attendance at Shelters, Soup Kitchens,Volunteer/Charity Sector locations, HMO accommodation and more.
Main duties of the job
1. Manage safeguardingconcerns appropriate to your role and level of training.
2. To plan, develop andfacilitate group and 1-2-1 sessions.
3. Offer comprehensive support to empower people totake control of their health and wellbeing. They will spend time with serviceusers to help them to focus on what matters to me and connect them tocommunity groups and statutory services for practical and emotional support. The role is not intended to support individualslong term, but to help them to understand how they can support themselves betterby exploring barriers to improved mental, social, and physical health.
4. Manage a caseload ofclients through assessment to onward referral, working with clients identifiedby the clinical team. They will provide ongoing support to promote engagementwith identified services and achievement of goals.
5. Contribute to the development of the service andparticipate in support, supervision and training as required. The role is anon-clinical role, however the post holder will receive support from a clinicalmentor.
6. Contribute to theeducation of staff within the network and maintain details and grow workingknowledge of sources of support in the community.

Job responsibilities
7. Develop trustingrelationships with service users, giving them time to focus on their assets andwhat matters to them and providing them with personalised help to takecontrol of their health and wellbeing, live independently and better understandthe impacts of their lifestyle choices.
8. Undertake holistic clientneeds assessments using a wellbeing template, pre and post interventions, tounderstand the impact on the clients wellbeing. Support, training andsupervision will be given.
9. Support a caseload ofclients for whom wellbeing support and mentorship might offer improvedoutcomes. With the team, proactively identify people who would benefit fromthis type of help and manage and regularly review your caseload to accommodateurgent referrals for support as required, referring out to community support servicesas soon as possible.
10. With clinicalsupport, develop a strong awareness and understanding of when it is appropriateor necessary to escalate care back to the GP or other care professionals whenthe persons needs are beyond the scope of the wellbeing mentor when thereis a mental health need requiring a qualified practitioner.
11. Following an agreedperiod of case management, refer clients on to the voluntary sector or localcommunity support services where appropriate. Make recommendations on where anon-clinical approach might support the patient better or complement existing clinicalinterventions and improve outcomes.
12. To be pro-active inmaintaining personal and professional learning and development and identify owntraining needs as they arise.
13. To attend teammeetings, away days and other similar events.
14. To participate fullyin regular one-to-one supervision sessions and in an annual performanceappraisal.
15. Work as part of theteam to seek feedback, continually improve the service, and contribute tobusiness planning.
16. Undertake any tasksconsistent with the level of the post and the scope of the role, ensuring thatwork is delivered in a timely and effective manner.

Essential
22. GCSE grade A-C or equivalent in English & Maths.
Desirable
23. Qualifications in health, social care or other related subject.
Experience
Essential
24. Demonstrable experience of working with people with multiple health and social needs within a health care setting, or community-based work with socially excluded groups.
25. Experience referring to local support agencies.
26. Experience of assessing need and working within time-limited interventions to promote and support change.
27. Experience of planning, facilitating and evaluating workshops / groups, focused on self-help.
Desirable
28. Experience of working with health inclusion groups including Asylum Seekers and Refugees, people experiencing homelessness and the Gypsy, Traveller, and Roma communities.
Special Knowledge & Skills
Essential
29. A sound knowledge of local support agencies and a commitment to the continual updating of personal knowledge in this area.
30. Excellent communication skills, both verbally and in writing.
31. IT literate, with a sound working knowledge of Microsoft packages and data management/case recording systems.
Desirable
32. The ability to speak an Eastern European, African, or Middle Eastern language.
33. Knowledge of how to work within the biopsychosocial framework.
34. An understanding of Trauma Informed Care.
