An exciting opportunity has arisen for a dynamic and enthusiastic trained nurse to join our established Coventry & Warwickshire Community TB Service. You will be working as part of the multi-disciplinary team providing high-quality TB care in collaboration with other services within both the acute and primary care settings to provide a seamless service for people receiving TB treatment, their contacts, and the wider community in accordance with nationally and locally agreed guidelines.
This is a fantastic opportunity for a suitable candidate wanting to develop their TB knowledge and to develop skills to become an autonomous specialist practitioner. You must have the ability to work both autonomously and as part of a multi-disciplinary team. The successful applicant will have their own caseload of patients and will run nurse-led TB contact tracing, BCG vaccination, and New Entrant Screening clinics. Excellent communication skills and an ability to work with people with diverse health and social needs are essential.
The post holder will work as a member of the Community Coventry & Warwickshire TB Service contributing to the delivery of an effective TB service assisting in the diagnosis, treatment, prevention, and control of TB.
Key Responsibilities:
1. Works collaboratively with the multi-professional team to develop and maintain a strategy relating to the care of patients in the specialist area.
2. Works both as an independent practitioner and as an effective member of the wider interdisciplinary clinical team. Demonstrates excellent communication ability.
3. Utilises an agreed service plan as a framework for service delivery.
4. Uses data from audit/quality monitoring/patient satisfaction questionnaires/service review to inform clinical governance. Utilises information to ensure that services are responsive to patient needs. Ensures patients are consulted in the development of services.
5. Is responsible for ensuring own clinical practice complies with Data Protection/Confidentiality/Caldicott principles.
6. Assists in maintaining standards of cleanliness in the clinical area and for a well-maintained ward/department environment.
7. Actively promotes diversity, including race diversity, both in the delivery of the Directorate’s services and in the management and development of staff within the clinical area. Generates and sustains relationships that promote dignity, rights, and responsibilities. Identifies and takes action to address discrimination and oppression.
The post holder will work across acute Trust/ICB settings, as an expert in the clinical field to deliver specialist clinical services to a defined patient/client group. They will assist in the development of the service in line with national and local targets and provide specialist, interdisciplinary education and training to other healthcare professionals & those working locally in relevant services.
The post holder will be supported through an in-house training programme to achieve clinical skills and knowledge competencies as outlined in the CNS job description. Once the in-house training programme is completed and competencies achieved, the post holder will be able to:
1. Hold their own defined caseload.
2. Communicate complex, sensitive information effectively and empathetically to patients and relatives. Maintain personally generated observations in records.
3. Assess, develop and implement evidence-based specialist nursing care programmes and advice for patients and relatives.
4. Work as an expert practitioner in the given area, acting as a resource for other interdisciplinary healthcare professions. Maintain a high profile in the clinical area, acting as a role model for good practice.
5. Work with professional groups and other agencies in both primary and secondary healthcare settings. Assume a central role in problem-solving and service coordination to ensure patients and carers receive high standards of seamless quality care.
6. When appropriate, participate in the establishment/running of nurse-led clinics. When trained, undertake diagnostic and therapeutic procedures pertinent to the specialist role.
7. Actively participate in interdisciplinary meetings.
8. Act on clinical incidents pertinent to the area, to reduce clinical risk.
9. Provide informal support and education to patients/carers and all professional disciplines.
10. Participate in the delivery and evaluation of formal education programmes related to the specialty.
11. Identify and plan education, participating in curriculum planning and delivery.
12. Conduct/facilitate primary research/contribute to research programmes. Promote evidence-based practice in the specialist area. Be aware of clinical trials that may affect patients in their own clinical area.
13. Provide formal mentorship and/or clinical supervision as required.
14. Contribute to the development of care pathways when necessary.
15. Develop/contribute to an information database of support services available to healthcare professionals and the patient group.
16. Use research findings critically to develop practice.
17. Monitor and maintain the health, safety, and security of self and any practitioners they are responsible for in the work area. Identify and assess risks in work activities, and develop strategies to manage these effectively. Practice within risk management/occupational health legislation and Trust procedures.
18. Develop their own skills, taking responsibility for continuing professional development and performance, maintaining their own portfolio in accordance with post-registration requirements. Work to achieve competencies appropriate to grade. Identify their own development needs in relation to current practice and future plans, setting personal development objectives.
19. Establish local and national networks, linking with other practitioners in the specialist area in order to share/learn from examples of good practice.
A full and valid UK driving licence with daily access to a car for work purposes is essential for this post.
