Devon Partnership NHS Trust is seeking a passionate and dedicated Band 5 Mental Health Practitioner to join our vibrant and supportive team in Tiverton. As a Mental Health Practitioner, you will provide high-quality care and support to individuals experiencing mental health challenges within our community services.
Responsibilities:
* Provide assessment, treatment, and support for individuals with mental health conditions in a community setting.
* Work closely with multi-disciplinary teams to deliver person-centred care, ensuring the best outcomes for individuals.
* Support the development and implementation of care plans, delivering therapeutic interventions and promoting recovery.
* Contribute to risk assessments and care coordination in line with best practice guidelines.
* Collaborate with service users, families, and external agencies to enhance community integration and improve mental well-being.
* Participate in supervision, training, and development opportunities to further enhance professional growth and service delivery.
Minimum Requirements:
* A Registered Mental Health Nurse (RMN), Occupational Therapist (OT), or Social Worker with a passion for supporting individuals with mental health needs.
* A commitment to delivering compassionate, person-centred care.
* Strong communication, organisational, and time-management skills.
* The ability to work effectively as part of a multi-disciplinary team.
* Experience in mental health or community settings is desirable but not essential.
* A proactive approach to continuous learning and development.
