Due to retirement, we have an exciting opportunity for a motivated andenthusiastic Practice Nurse/ HCP with an interest in chronic disease management to join our friendly, patient focused team.
This role will primarily be responsible for chronic diseasemanagement including the diagnosis, monitoring and development of patient'sindividual management plans using evidence-based practice.
Depending on hours, the role may also include treatment room responsibilities: immunisations,, cervical screening, wounds and supporting the team in the reviewing and delivery ofclinical policy and procedures
Main duties of the job

Working as a senior member of the practice multidisciplinary team,the post holder will ensure long term condition management is delivered effectively to theentitled patient population.

Job responsibilities
Primary Responsibility
The key responsibility for this role is chronic disease management including the diagnosis, monitoring and development of patients individual management plansas a senior member of the practice multidisciplinary team.
1. To ensure nursing services are delivered effectively to the entitled patient population.
2. To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice.
3. Implement and evaluate individual treatment plans for chronic disease patients.
4. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health.
5. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF.
6. Request and process pathology results as required.
7. Maintain accurate clinical records in conjunction with existing legislation.
8. Ensure read codes are used effectively and support with achievement of QOF targets.

12. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).
13. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
Secondary Responsibilities
14. Provide wound care (ulcer/Doppler etc.) to patients.
15. Provide immunisations (childhood, seasonal etc) to patients.
16. Provide Cervical Cytology screening.
17. Chaperone patients where necessary.
18. Support the team in dealing with clinical emergencies and safeguarding matters.

Essential
19. Experience of working in primary care
Desirable
20. Experience of managing long term conditions - Asthma, COPD, Diabetes.
21. Vaccination and Immunisation trained.
22. Wound management including Dopplers.
23. Experience of TPP SystmOne.
