Job summary An exciting opportunity has arisen for an enthusiastic and experienced childrens nurse to join a supportive, dynamic team within the Paediatric Diabetes Specialist Service across County Durham and Darlington. The successful applicant will work within the Paediatric Diabetes Team, consisting of Consultant Paediatricians, Lead Nurse, Specialist Nurses, Specialist Dietitians, Psychology, HCA and admin, all who are passionate about improving outcomes and experience for children and young people with Diabetes. Joining a well-established, supportive service, the successful applicant will be an active member of the Paediatric Diabetes MDT for children and young people diagnosed with Type 1 and Type 2 diabetes, providing high quality care in line with local and national guidance, and the Best Practice Tariff. Main duties of the job We are offering this rewarding position in transitional care to make a meaningful contribution to the lives of young people aged 14-19 years with diabetes. In line with the national transition agenda, the successful candidate will support and empower young people to manage their Diabetes, as they prepare to transition into adulthood. The post holder will also contribute to the wider Paediatric Diabetes for all ages. The post holder will support the MDT diabetes team providing high quality, evidence based care to children and young people and their families with diabetes in day to day care, offering support and advice to optimise the control of their blood glucose and their physical, psychological, intellectual, educational and social development. The role spans outpatient settings, inpatient and assessment areas, community and requires collaborative working across the organisation and wider system.
