Job overview
An exciting opportunity has arisen for a Senior Community Falls Nurse to join The Falls Prevention Service in Waltham Forest, East London.
The successful candidates will be expected to support their team, department and organisation to achieve the Trust’s Values in their day to day work. They will be a Registered Nurse (RN12-NMC) responsible for patients allocated to them and will assess, manage, plan and deliver Falls care, including completion of a Community Falls Assessment, the provision of exercises & basic equipment as well as being professionally and legally accountable.
The successful candidates will support their peers, team leader and patients in Nursing Care, as an effective team member whilst working on their own initiative. They will supervise pre-registration students and Band 3/4 Rehabilitation Assistants. They will have a commitment to collaborative working actively supporting and liaising with other health and social professionals and agencies in the nursing management of clients. They will provide Falls Education to patients in the community or at our Falls Education Classes.
Main duties of the job
The post holders will lead the nursing management of Waltham Forest Falls Prevention Service Users across Waltham Forest. They will be responsible for the management of a caseload ensuring the provision of a high standard of nursing care to patients and their families in the community. They will work as part of the Waltham Forest Falls Prevention Multidisciplinary Team, liaising with GPs, other professionals and statutory and voluntary agencies to deliver individualised care to patients. They will work without direct supervision and provide leadership to the Waltham Forest Falls Prevention Service. They will complete Community Falls Assessments and provide Falls Education, exercises and basic equipment to patients, as well as nursing care.

Probationary Period: This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Starting with NELFT: NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.

Experience
* Experience of working in a Multidisciplinary team
* At least 2 years post registration experience
* Community Nursing Experience
* Experience in long term conditions & rehabilitation
Knowledge
* An awareness of NHS priorities
* Knowledge and skills to assess patients with complex presentations to arrive at clinical diagnosis
Skills
* Proficient in IT skills
* Excellent communication & interpersonal skills - verbal & written
* The ability to identify & act upon adverse clinical signs
* Ability to work under pressure & prioritise caseload to respond to timely referrals
