We are looking for someone who is experienced in mental health services.
• To work as part of a team that provides mental health services, to positively promote recovery and social inclusion by focusing on the direct needs of service users, working across boundaries of care, organisations and roles
• To provide support directly and flexibility to service users towards goals defined in his or her care
• To create specific care plans aimed at promoting social engagement, healthy living and independence
• Maintain information and knowledge of and links with community resources and actively support service users to use these resources, thereby countering discrimination and encouraging social inclusion
• Planning and organisation of activities, care plans and programmes of care for Service Users under the Community Mental Health Team in collaboration with the Service User, Carer, Care Co-ordinator
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
