Job Description:
We are currently seeking a passionate and dedicated Adult Social Worker to join our Adult Integrated Multi-Disciplinary Hospital Team. As an Adult Social Worker at Hull City Council, you will assess the needs of adults requiring social care support, develop care plans, and coordinate services to promote independence and improve quality of life. You will collaborate with service users, their families, and other professionals to deliver person-centred care and support.
Key Responsibilities:
* Conduct assessments of individuals’ care and support needs.
* Develop and implement care plans in collaboration with service users and their support networks.
* Liaise with external agencies and professionals to coordinate services and support.
* Provide advice, guidance, and advocacy to individuals and their families.
* Monitor and review care plans to ensure they remain effective and responsive to changing needs.
* Participate in multi-disciplinary team meetings and case conferences.
Requirements:
* A recognised qualification in Social Work (e.g., Degree in Social Work, DipSW, CQSW).
* Registration with Social Work England.
* Experience working within Adult Social Care, preferably within a locality or community setting.
* Strong assessment and communication skills.
* Ability to work effectively within a multi-disciplinary team.
* Commitment to promoting independence, dignity, and empowerment for service users.
