Job Title: Adult Social Worker – Community Team – Newquay

Job Description:
1. To work with other team members and multi-disciplinary team members from other agencies to meet the needs of vulnerable adults, operating as an effective member of the operational team ability to be based or travel to a required location.
2. To ensure the views and wishes of individuals and carers are given appropriate consideration and are central to all decision making, ability to complete MCA and Best Interest decision making.
If required to complete COP applications and attend hearings.
3. To act to protect adults in need of support services in line with legal requirements, ability to complete safeguarding enquiries, self-neglect intervention.
4. To act within a framework which promotes independence, prevention, re-ablement and self-determination, facilitating effective communication with families, carers, individuals, and other agencies.
5. Any special requirements – Essential to be based within travel distance of Watergate PCN or ability to travel as required daily into the area.
Requirements:
You will be a social worker qualified with a Social Work Degree, Social Work Qualification (Dip. SW) or equivalent and registered with Social Work England, who will have:
6. Expert knowledge and experience of relevant legislation, social work theory, policy and principles and evidence of these in your practice.
7. Previous experience of working in a social care environment with a proven track record of excellent strengths-based practice.
8. Ability to undertake, analyse and translate complex assessments of need, risks, and strengths.
9. Proficiency at recognising and responding to behaviour that may indicate resistance to change, ambivalent or selective cooperation with services, and recognise when there is a need for immediate action. You will be able to deliver difficult messages.
10. Ability to form strong professional relationships with other partners within a multi-disciplinary setting.
11. A high level of written and verbal communication skills.
