Job summary The role of the community LD nursing team is to support the holistic delivery of care to service users across a range of needs. The role of a community learning disability nurse involves improving or maintaining a persons health, reducing barriers to them living an independent life, supporting the person in living a fulfilling life, improving the well-being and social inclusion of people with a learning disability, offering help and support to their family and carers and health facilitation. As a band 6 community learning disability nurse, your role will involve completing the following tasks: To work autonomously taking responsibility for care, quality and clinical activity for a designated caseload / team. To contribute to care quality performance and clinical activity by providing professional nursing leadership and guidance, clinical advice, and management in relation to the key dimensions of health care quality. Act as a professional and clinical role model to all staff, patients, carers and the public at all times Supervise the performance standards of own team members where appropriate. Maximise use of clinical and staff resources, implementing best practice in patient pathways, admission and discharge arrangements. Plan and promote the development of new services within the designated area. Maintain efficient and effective communication on issues related to patient care, caseload and organisation management. Participate in staff recruitment and selection processes and wider recruitment and retention strategies. Act in a professional manner and ensure self and colleagues adhere to the LPT Uniform Policy at all times.
