1. To provide a specialist and advanced nursing contribution to the clinical management of patients with breast cancer across all boundaries.
2. This 12-month developmental post will aim to provide a specialist Nurse led service for patients with a Family History of Breast Cancer and Breast Pain.
3. In addition to providing a specialist Nurse Led Follow up Service for Breast Cancer and Family History patients.
4. Supervising and leading the team with the patient centred follow up management will also be an essential aspect to the role.
5. The post also incorporates development in leadership and management.
6. On completion of successfully meeting training requirements and a competency framework, a Band 7 senior clinical nurse specialist post will be offered.
7. The Clinical Nurse Specialist will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout the care treatment pathway and after care support.
8. The Clinical Nurse Specialists will ensure the coordination and management of patients at all points of their treatment but in particular will also act as a point of contact for patients who will have been prepared by their clinical team for Patient Led Follow Up (PLFU) - and will be able to sign post patients to support their self-management
1. To act as a resource for the multi-professional teams involved in the care and management of patients with a breast cancer diagnosis.
To maintain and develop the Breast Care Nursing service at Walsall Healthcare Trust, establishing good working relationships with clinicians and other key staff.
To be part of the multi-professional breast cancer tumour group within the Trust, Network and Region (where required) developing standards of care for patients diagnosed with breast cancer.
To provide an effective and efficient service, delivering specialist advice and supportive care to patients who have, or fear they may have, breast cancer, and their families.
Contribute to Breast care follow up with the focus on the physical, sexual, psychological and social issues of care provided to the patient, through specialist skills, motivation, vision, up-to-date knowledge and clinical practice
To attend and be a core member of the Breast MDT meetings in accordance the national guidance.
To manage own workload across a wide range of specialist activities which includes organising case conferences, planning throughout the patient pathway, and directly liaising with other healthcare professionals and agencies.
Assist with the implementation of Treatment Summaries
Develop and implement personal treatment plan which puts patients in control of their own follow up, together with suitable literature/handouts/posters.

1. The post holder will work independently as a Clinical Nurse Specialist to ensure:
2. The coordination and management of patients undergoing treatment at Walsall Manor Hospital to support patients both on and off treatment.
3. Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers.
4. Provide telephone advice and triage where appropriate, working within defined protocols and guidelines. Document and communicate outcomes as per service specification.
5. Provide level 2 psychological support within own knowledge boundaries following appropriate training.
6. Perform Holistic Needs Assessments using designated screening tools as appropriate
7. Collate patient surveys/patient experiences surveys and develop action plans.
8. Develop and facilitate Health and Well Being Programmes/Beyond Cancer. This will involve contributing and delivering education to patient and carers, including information around health promotion, Breast Cancer information and supporting self-management.
9. Document all interactions both verbally and written with accuracy and in line with Trust policy and procedure.
1 Manage existing follow up clinics and create new clinics within the self-supported management structure and the Macmillan vision.
