Middlesbrough Borough Council are looking for a dedicated social worker to join their Transformation Learning Disabilities Team on a full time basis
Responsibilities
Conduct assessments of individuals with learning disabilities to identify their needs and develop personalised care plans
Manage a caseload of individuals, ensuring regular reviews of their care and support plans, and coordinating services to meet their needs
Ensure the safeguarding of vulnerable adults, addressing any concerns related to abuse, neglect, or exploitation in compliance with legal frameworks
Work collaboratively with health professionals, families, carers, and external agencies to provide holistic support and improve outcomes
Support individuals to live as independently as possible, facilitating access to employment, education, and social activities
Provide timely interventions during crises, offering support and guidance to both individuals and families during challenging situations
Ensure all work is in line with local authority policies, procedures, and legislation, including the Care Act and Mental Capacity Act
Requirements
Registration with SWE
Qualification in Social Work
Minimum 2 years experience as a social worker
