This is an exciting opportunity for an individual interested in palliative and end-of-life care to join our dynamic specialist team. This temporary post covers maternity leave.
We seek an enthusiastic, highly motivated Registered Nurse to join our multi-disciplinary team in providing specialist palliative care to patients and their families.
The Community Team works within a Clinical Hub structure, and the Clinical Nurse Specialist will collaborate with other Clinical Nurse Specialists, a Senior Healthcare Assistant, and a Hub Clinical Lead.
The Clinical Nurse Specialist will manage patients with complex needs, assessing and planning care to meet practical, psychological, spiritual, and social needs. They will work closely with primary and secondary care professionals and Hospice colleagues, making good communication and teamwork essential.
While the role can be busy and challenging, it is rewarding to improve the quality of life for those with life-limiting conditions.
