Job summary Warrington Recovery Team (Adult Community Hub) are looking to recruit a dedicated and committed B6 Senior Nurse Practitioner to come and join our wonderful, caring and very supportive Team. We are looking for an enthusiastic B6 practitioner to work as part of our routine pathway function, undertaking assessments of need for service users who have been referred to our Team. You will work alongside experienced colleagues, to help identify the most appropriate pathways and interventions for people and help signpost onto other services and support. This is a very rewarding job, with clear, time focussed outcomes. Previous experience of a similar nature to this role is important, although you will be offered additional training and ongoing support, to develop in this role. We are a highly regarded, well led multi disciplinary service, providing excellent care, treatment and support to adults with significant mental health needs, throughout the Borough of Warrington. Our office base is Wakefield House and our office hours are currently Mon to Fri 9-5. Main duties of the job This B6 post is specifically to help support our routine function, undertaking assessments of need for service users who have been referred to Team, identifying longer term care and support needs and signposting onto appropriate services. The role will require you to liaise with other agencies and professionals. You will also work closely with carers and families, alongside our wider partner agencies, to achieve best outcomes for our service users. The Team has a strong value base, centred on caring and commitment. You will receive ongoing clinical and management supervision and have opportunities for CPD as part of this role.
