Job summary We are recruiting a full time Recovery and Wellbeing Worker to join our Step Down Team in Wells to support individuals leaving hospital to transition back to independence. The Step Down Service will work with clients on their journey from acute services to their own accommodation, coproducing strength-based, individual recovery plans that provide strategies to; better manage their mental health, identify warning signs, build resilience and nurture positive relationships. From the moment someone moves in, the focus will be on life after move on and building practical living skills to support independence and wellbeing. Proven experience of working within a housing environment. Experience of working with people who have experienced multiple disadvantages (e.g. mental health, homelessness, alcohol/drug dependency long term conditions) Experience of working with people with long-term mental health and complex needs including complex trauma, Learning Difficulties, neurodiverse needs and SEND. Experience of carrying out needs and/or risk assessments and the support planning process. You will be practiced at working alongside people using a holistic approach tailored to each individuals needs Experience of time management and competing priorities when supporting people with complex needs.
