Key Responsibilities: The post holder will be responsible for triaging, assessing, planning, and delivering complex care packages to admission-vulnerable service users across Devon. They will also ensure proactive liaison with other partner agencies, families, and care providers at every stage of care.

What We Are Looking For: We are looking for an experienced clinician with a strong background in mental health and learning disability nursing, occupational therapy, or speech and language therapy. The ideal candidate will have excellent safeguarding, Mental Health Act, and Mental Capacity Act knowledge, as well as advanced interpersonal, communication, formulation, and risk assessment skills.

Salary Details: This permanent position comes with a competitive salary range of £46,148 - £52,809 per annum.
