Summerhill Surgery is a well-established local GP surgery in the Kingswinford area.
We are looking for an additional member to join our Practice Nursing team due to an increase in registered population.
The post holder is responsible for the delivery of basic practice nursing services care to the practice population. Supported by senior nurses within the Practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities.
They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required.
Main duties of the job
1. Assist with the organisation and coordination of the provision of nursing services for the practice.
2. Provide nursing treatment to patients in participation with general practitioners or independently agreed protocols, including cervical smears.
3. Provide general and specific health screening to the practice patients (within the agreed protocols) with referral to general practitioners as necessary.
4. Be competent in administration of childhood immunisation and travel vaccination.
5. Be competent and confident in managing people with long term health conditions.
6. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
7. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
8. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
9. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
10. Implement and participate in vaccination and immunisation programmes for both adults and children.
11. Meet the needs of patients presenting for opportunistic wound care.

Summerhill Surgery is a happy surgery trying to provide the best possible service and have a patient-focused ethos.
We currently have 7300 patients with approximately 15% living in Care Homes who are looked after by our specialist team.
We have 3 GP Partners and a non-clinical Managing Partner with 3 further salaried GPs. Other members of our Clinical team include ANPs, Nurse Practitioners, Practice Nurses, HCA's, a GP Assistant, a Paramedic and Pharmacist support. We also have a large hardworking admin team providing support to the clinicians and patients.
Job responsibilities
To work as part of the team responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
This organisation will ensure that the post holder has access to appropriate clinical supervision and support on a day-to-day basis.

Qualifications
* A Registered Nurse
* Experience working in a primary care environment
* Experience working autonomously
* Understanding the importance of evidence-based practice
* Ability to record accurate clinical notes
* Ability to work within own scope of practice and understanding when to refer to GPs
* Clinical knowledge in the following:
* Wound care/removal of sutures and staples
* ECGs
* Venepuncture
* New patient medicals
* Immunisations (routine, childhood and travel)
* Requesting pathology tests and processing the results, advising patients accordingly
* Chronic disease management awareness for:
* Diabetes
* CHD
* Asthma
* Specialist interest in at least 1 Chronic Disease, e.g. Asthma, COPD or Diabetes
* Experience of managing elderly and housebound patients
* Experience using the EMIS clinical system
* Experience with clinical risk management
