Support Coordinator
This is an exciting opportunity to work with stroke survivors and their families to support them following stroke. We are looking for an innovative, passionate, and professional individual with excellent interpersonal and organisational skills to join the Stroke Recovery Service based in Hertfordshire.
Position: S11261 Stroke Support Coordinator
Location: Home-based, Bedford. However, frequent travel will be required as part of this role (to include team meetings or other work-related meetings).
Hours: Part-time, 28 hours per week
Salary: GBP21,391 per annum (FTE GBP26,700 per annum)
Contract: This is a fixed term contract until 31 March 2026. Our services are contracted, and we currently have funding for this contract until 31 March 2026.

The Role
Reporting to the Service Delivery Coach in the East of England, you will support stroke survivors in identifying their goals using strong assessment skills and a person-centred approach, enabling them to maximise their recovery and improve communication.
Key responsibilities will include:
1. Supporting new stroke survivors and their carers from hospital discharge into the community.
2. Supporting a diverse caseload including anyone experiencing communication difficulties via a combination of face-to-face visits or meetings, telephone calls, emails or letters, and digital methods (such as video calls).
3. Providing personalised information, advice, and support.
4. Enabling stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
5. Working with other health and social care professionals across the stroke pathway to ensure high-quality support.
About You
You will have experience in:
1. Experience/background in a caring profession, ideally supporting people with disabilities.
2. Excellent IT skills and an ability to maintain accurate records.
3. An affinity with the values of the Stroke Association.
4. A flexible approach and an ability to effectively manage a caseload.
This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.
