The Central locality Community Mental Health Team within Rotherham, Doncaster and South Humber NHS Foundation Trust has an exciting opportunity for a Band 5, CPN to join their team. The Central locality team provides support to individuals with complex and enduring mental health issues. The team offers a range of management strategies and interventions including various psychological therapies. Care and support is offered in line with the CPA approach. Applicants must demonstrate a flexible approach, evolving around the needs of service users, their families and significant others. The role involves working as a team member, with the responsibility of a caseload across traditional boundaries of Adult and the voluntary sector services. We are looking for an enthusiastic person with excellent team working skills, who is keen to develop their knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience. Job Summary To carry out nursing interventions within the community as required. To assist in the development of the Nursing Team, working with other agencies to ensure the highest standard of patient care. PRINCIPAL DUTIES / MAIN TASKS. Managing a team caseload under supervision in accordance with sound nursing practice and principles. Undertaking regular supervision and PDR in accordance with Trust policy. Promoting and Maintaining the highest standards and attitudes towards the care of patients / clients. Assisting colleagues in maintaining an effective and efficient service. Assisting, developing, implementing and evaluating plans of care, ensuring therapeutic and recreational programmes for patients / clients individually and in groups, in the Community. Ordering, storage, monitoring and administration of medication.
