Our therapy team works across multiple locations in Solihull, providing vital services to individuals who need further assessment and rehabilitation after being placed in discharge-to-assess beds or returning home for continued care. We focus on patients with complex discharge planning needs following hospital admissions.
This role involves providing physiotherapy services in patients' homes and working closely with community services including the community therapy team. We are committed to fostering a positive culture that supports professional and career growth.
The position is part of a 7-day therapy service, which includes weekend shifts. The role also involves being part of the respiratory on-call list for UHB hospitals. Working hours are 37.5 per week, with shifts including 7-3, 8-4, and 10-6, covering weekends and bank holidays.
This opportunity is ideal for an experienced Band 5 professional looking for career progression, or a Band 6 professional wanting to collaborate with a dedicated team on complex patient care. A full driving licence and access to a vehicle are required.
Main duties of the job
1. To deliver a high standard of therapy provision within the Solihull Community and Intermediate Care Bedded Units across Pathway 1 and 2.
2. To support our patients in their recovery journey and minimise the risk of hospital readmissions.
3. To make the transition from acute care to the community as smooth as possible, ensuring timely discharges and efficient flow.
4. To liaise with various agencies to ensure safe, effective and efficient discharges.
5. To provide Physiotherapy services in the patient's own home or residence.
6. To contribute to seven-day working programmes across the organisation.
7. To be part of On-call respiratory Rota.
8. To be able to manage your caseload and work independently.
9. To supervise students on placements.
10. To appraise, supervise and support junior staff and therapy assistants.

Qualifications
Essential
* Good standard of general education including GCSE English and Maths grade C or above or equivalent
* A Levels or equivalent qualification
* Degree/diploma in Physiotherapy
* Registered with Health and Care Professionals Council
* Evidence of CPD including recent post-graduate training relevant to clinical speciality
Desirable
* Evidence of leadership training
* Fieldwork Educator's certificate or attendance at Clinical Educator Course
Experience
Essential
* Demonstrates clinical experience enabling the post holder to manage a caseload of patients with complex needs.
* Post grad Experience in Medical, Older People, Trauma and Orthopaedics & Respiratory
* Specialist knowledge across a range of practices, routine and non-routine, requiring expertise within the field of Physiotherapy relating to Community Services
* Previous and ongoing attendance of mandatory training
* CPD portfolio
Desirable
* Experience and understanding of team/service development
* Supervision of Band 5 staff, students and assistants
* Clinical experience within Community Therapy Services including admission prevention service
* Previous and ongoing attendance at recent postgraduate courses relevant to the specialist clinical work

* Where the post includes a requirement to travel the post-holder will be able to meet the travel requirements of the post, and as such it is essential to possess a full category B car driving licence preferably clean or with no more than a maximum of 3 points
* Business insurance for a car/Vehicle.
