HC-One has developed a Specialist Dementia Care Community (SDCC) at Meadow Bank care home. Sabrina House has provided care for people living with dementia for many years, and following significant investments in environmental layout, design, workforce, and learning & development, we have exciting new opportunities for those passionate about supporting people who are emotionally distressed by their dementia and need a specialist service to meet their needs.
Sabrina House SDCC is a therapeutic care setting that emphasizes quality of care and quality of life equally. It offers a bespoke living environment with space, calming de-escalation rooms, enhanced staffing, and specialist training for colleagues in positive behavior approaches, psychosocial therapies, de-escalation, and safe restrictive practices. Our goal is to understand residents' needs as individual, not just their condition.
Supporting distressed individuals involves providing quality and purposeful daily life through personalized lifestyle plans, using a person-led, strengths-based approach with life story work.
We seek colleagues with patience, knowledge, and a compassionate attitude to understand residents' needs, find their 'why', and support them to live their best life.
The role of Therapeutic Wellbeing Practitioner is rewarding. You will support residents to live meaningful, fulfilling lives through tailored activities, personalizing their rooms, fostering relationships, maintaining interests, and encouraging independence, as well as engaging in spontaneous conversations and fun.
You will be responsible for the effective delivery of assessment, planning, implementation, review, and evaluation of wellbeing activities, using a bespoke Wellbeing Assessment Tool. You will attend Case Review Meetings to report on residents' Wellbeing Plans.
Your enthusiasm and creativity will inspire colleagues to participate in residents' wellbeing, supporting them to live the lives they want while receiving appropriate care. You will be a role model, promoting wellbeing as a core part of everyone's role.
We are looking for candidates with experience working with vulnerable individuals, preferably with activity program development, dementia care, and wellbeing knowledge, recognizing the importance of meaningful occupation and social interaction. Your positive energy, communication skills, and kindness are essential.
In return, you will gain access to a variety of benefits supporting your physical and mental health, along with ongoing investment in your development as a Therapeutic Wellbeing Practitioner.
Our sector-leading Dementia Care Learning and Development programme covers communication, empathy, relationships, inclusion, complex needs, and more, delivered through e-learning and face-to-face sessions, enabling you to become a specialist in dementia care.
We support your career growth and offer a fulfilling career path with us, including benefits such as:
* Competitive hourly rate based on experience and qualifications
* Paid Enhanced DBS/PVG
* Free uniform
* Nutritious meals during shifts of eight hours or more
* Company pension scheme
* 28 days annual leave including bank holidays

HC-One values kindness, integrity, prioritization, clarity, and value in all actions. We believe our residents are individuals with stories to share, and joining us offers a chance to make a meaningful difference.
