This role will be providing unplanned nursing care for patients across East Exeter and Mid Devon out of hours.

Are you passionate about providing high quality nursing care to people in their own homes, supporting them to remain in their preferred place of care preventing unplanned admissions? If you are looking for a new challenge, adaptable, and able to think on your feet and outside the box, then the Out of Hours Nursing Team is for you!
The Out of Hours Nursing Team consists of nurses and healthcare assistants. The caseload covers a broad range including end of life care, admission avoidance, support for timely hospital discharge, and urgent unplanned nursing needs.
You will be able to demonstrate your enthusiasm and commitment to community nursing, and further develop your clinical skills in meeting the varied needs of patients out of hours. The post-holder will be supported by Band 6 nurses, the Team Manager, and Clinical Matron, and will work with an HCA on all clinical visits.
Applicants will be required to commit to both shifts, however consideration will be given to applicant preferences and work-life commitments. The team have use of Trust vehicles for home visits.
We support and encourage additional learning to develop your clinical knowledge base. On appointment to the team, you will be assigned a buddy who will support you on your journey within the OOH Nursing Team.
Experience in community nursing is desirable for these posts but not essential as full training/support will be provided, and the post holder will be supported to develop in the role. There will also be many opportunities to develop and progress your career.
Working Pattern: 37.5 hours per week between the hours of 16:00 to 08:00, worked over 2 shift patterns 16:00 to 00:00 and 22:30 to 08:00.

Pay: The salary advertised for this position reflects the Agenda for Change 2024-5 rates, inclusive of a 5.5% pay award implemented from October 2024. If you join before this date, the pay award will be backdated to April 2024 or your start date, whichever is later. Until then, remuneration will be based on the 2023-4 pay bandings.
