We are looking for an enthusiastic and adaptable Urgent Care Service Nurse to join our strong and progressive team in Folkestone. The Urgent Care Service in Kent is encouraging innovative change to ensure that patient care is holistic and needs-led. We are a consultant-led team of Doctors, Advanced Clinical Practitioners, Occupational Therapists, Physiotherapists, Nurses, and Health Care Assistants specialized in delivering short-term hospital-level care to people in the home environment. Community is the heart of our trust and we are working with primary care colleagues locally to respond to the needs of our patients and we want you to be part of this change.
We aim to respond to referrals within 2 hours and will arrange to visit to assess, diagnose, treat, and implement care strategies that will enable our patients to remain safe and well in the comfort of their own home. Folkestone is made up of a diverse patient group with complex needs and by working in a community-based way we can ensure that services are targeted and patient-focused, and we are proactive in our input and response.
Main duties of the job
You will play a vital role in our multidisciplinary services; managing patients with complex needs in the community. You will do this by completing comprehensive frailty assessments and working with our patients and their families to build clear plans based on current healthcare needs but also the wishes and preferences for future care.
You will have the freedom to plan care in your own way, with time to develop your assessment and treatment skills, and access to structured supervision and a clinical lead to ensure you receive the support necessary.
You will be supporting junior colleagues with clinical development, helping them to reach their potential and develop the workforce of the future.
You will be required to drive between patient homes and work sites and will need to provide your own transport to achieve this.

Person Specification
Qualifications
* Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC or Physiotherapist; Occupational Therapist; etc / HCPC). Must have valid UK NMC PIN.
Experience
* Experience of recent community/acute healthcare gained through education, work experience placement, or previous employment.
Knowledge
* Awareness of professional issues and recent developments in the NHS; knowledge of up-to-date clinical practice.
Skills and Abilities
* Sound communication and interpersonal skills (verbal and written); ability to organise own work; ability to cope with change; ability to use own initiative; holds current UK driving licence & access to a vehicle.
Personal Attributes
* Ability to motivate and organise others to ensure best practice; ability to work under pressure; flexible and adaptable.
Ability to Manage
* Able to support the implementation of local and national agendas for health under the guidance of the team lead/ward manager; ability to prioritise care delivery in a professional manner, considering Trust policies and procedures.
