FDIDT are anin reach multidisciplinary team made up of AHPs Nurses and AssistantPractitioners. This Nottingham CityCare Partnership service providesinnovative successful and patient focussed outcomes for those that present inthe emergency and urgent pre admission areas at Nottingham University HospitalsQMC site.
Asthe Team Manager you will be responsible for leading the busy and dynamic team. The role includes delivery of efficient and effective services in line withlocal national and professional agendas. This involves line managing fifteen plus staff, monitoringperformance, managing change and ensuring contractual targets are met.
The successful candidatemust demonstrate good operational management ability to facilitate servicedevelopment, manage competing demands and have excellent leadership skills. Change management is also a key requirement as the services continue to transformwhile we move forward with the health and social care agenda. A good understanding and experience of Healthand Social care provision and ability to work closely with partnerorganisations and other stakeholders is essential.
Main duties of the job
To work with the ICB Commissioners Integrated Health andSocial care managers and the Urgent care system to influence and implement thedirection of travel across service functions reflecting local and nationalpolicy direction.
Todemonstrate operational output from strategic direction and take theorganisation forward in todays commercial market by influencing andimplementing service transformation and improved patient outcomes, beingprominent in the leadership of the cultural change required to transform thedevelopment and delivery of community health services and build healthiercommunities.
To ensureperformance targets and strategic objectives are translated into operationalrealities and are embedded in staff roles and functions, therefore supportingCityCare as a key player in the provider area.
Bring alivethe clinical workforce planning activities and the modernisation and re designagenda amongst front line staff.
To develop with other senior directorate managersappropriate mechanisms and structures for the planning and implementation ofservice expansion and ensure services are designed around service users andcommissioners.
Developnetworks through working in partnership through the ICB Urgent Care system andother stakeholders encouraging contribution and creating an environment whereothers have the opportunity to contribute.
Toparticipate and lead in conjunction with business partners in contract servicereviews and service negotiations.

Job Purpose
1. To provide effectiveleadership and management support to the Hospital based Front DoorIntegrated Discharge clinical team.
2. To drive Changemanagement through the service functions within established service practiceand across new business opportunities with the focus on admission avoidance aspart of the urgent care agenda, prevention of long stays and earlyidentification of needs through holistic assessment
3. To develop a highperformance team culture aligned to organisational centric objectives, focusand behavioural outcomes across the service line and through strategic andoperational plans.
