Assistant Practitioner Community Therapy - Grimsby

We are looking for an experienced Therapy Assistant or existing Assistant Practitioner to take up this role within the Community Therapy Team in North East Lincolnshire.
This team sees a range of patients within the community, in their own homes, and at a range of care facilities including a drop-in assessment clinic to trial equipment and assistive devices to aid independence. Your role will involve assessment to provide appropriate intervention for these individuals to aid maintenance of safe independent function in their homes. You will cover the drop-in clinic and see patients at home.
Individuals can present to the service with a range of different presentations and diagnoses, and interventions could include specific equipment and some rehabilitation plans with exercise and functional practice to improve mobility, balance, range of motion, strength, and thereby impact on their daily routines, and potential to consider major adaptations where required.
Main duties of the job
The focus of the role is to assess and support individuals to facilitate safety and function in their home environment. Interventions can therefore include a range of assessments and rehabilitation activities, assistive equipment, assessment for equipment, advice and information sharing, and assessment for major adaptations.
Excellent communication skills are required to ensure a collaborative approach to patient care from the multidisciplinary team involved. The post holder will be required to hold a Foundation Degree in Professional Practice or an equivalent Level 5 qualification in a relevant subject area. The purpose of the post is to provide a high standard of therapy service working under the supervision of registered Occupational Therapists and Physiotherapists.

Person Specification
Education and qualifications
* NVQ III Diagnostic and Therapeutic Support or equivalent
* Current full UK clean driving license
* IT related skills or training in Direct Care
* Previous experience in specialist area of post
* Other training related to post or clinical area such as Child Protection, Orthotics, sports and leisure, swimming qualification
* Previous experience of multidisciplinary and/or multi-agency working
Occupational experience
* At least 2 years working as an OT or physiotherapy assistant
* Or at least 5 years relevant work experience
* Have an understanding of patients who are frail, elderly and have multi pathologies including mental health problems
* Awareness of DFG processes
