With TWO opportunities available, we are seeking passionate and dedicated Registered Nurses to join our Ambulatory Care team! We will also consider job-share arrangements.
Permanent full-time - 38 hours per week
Permanent part-time - 32 hours per week
In this role, you will focus on reviewing patient files, identifying frequent presenters to the emergency department, and linking them to the appropriate services to support their long-term health and well-being. We are committed to providing compassionate and coordinated care to individuals with chronic and complex needs. The Connecting Care Service is dedicated to offering care coordination, self-management support, and health coaching for individuals in the community to help maintain their wellness and reduce unnecessary hospital visits.
Key responsibilities:
* Conduct in-depth assessments of patient records and histories to identify gaps in care or services
* Assist patients in accessing services they need to reduce hospital visits, providing support for chronic conditions, mental health issues, and other factors that impact their overall health
* Work with multidisciplinary teams, including physicians, social workers, and community support services, to develop and implement care plans for patients
* Provide home visits where necessary, ensuring patients receive the care and support they need to manage their health in the community
* Ensure patients are receiving the care they need, including transport assistance and appointment scheduling
You will be a current Registered Nurse with AHPRA who:
* Can work autonomously and manage a diverse caseload of patients with varying needs.
* Has strong verbal and written communication skills to liaise with patients, families, and a variety of health professionals
* Is comfortable working collaboratively with a multidisciplinary team to achieve optimal patient outcomes
* Is committed to providing compassionate care and working closely with patients and their families to achieve the best outcomes
