An exciting opportunity has arisen for a Diabetes Specialist Nurse to work within our Diabetes specialist nurse team at South Tyneside and Sunderland NHS Foundation Trust. The diabetes specialist nurse will work within a well-established diabetes specialist nursing team delivering patient care, leading the management and care delivery for patients diagnosed with Diabetes. The Diabetes specialist nurse role will consist of providing expert clinical knowledge to patients with Diabetes and supporting those patients who are newly diagnosed with Diabetes. There will be a requirement to contribute to the ongoing development and delivery of nurse-led services for patients.

Main duties of the job
* Ensure that safe, effective and holistic patient care is delivered, and coordinate patients' overall plan of care throughout the patient journey.
* Assess and plan patients' care needs in an accurate and timely way, implement and evaluate the planned care using a holistic approach.
* Identify any additional needs of the patient/carer and refer to an appropriate member of the multi-agency/multi-professional team via the agreed pathways, making reasonable adjustments to maximise the patient's experience.
* Accurately observe and record patients' clinical condition, and initiate appropriate action or escalate as required.
* Accountable for the safe administration of drugs in accordance with NMC guidelines for administration of medicines and STSFT Policies.
* Accountable for accurate nursing records kept in line with NMC guidelines on records and record keeping, and STSFT record keeping policy.
* Promote excellence in nursing care, and demonstrate evidence-based best practice.
* Undertake diagnostic tests as required.
The successful candidate will be expected to work across various Trust sites.

Job responsibilities
A specialist nurse is defined as a qualified nurse whose primary role is to provide care, guidance and support which is based on an advanced level of knowledge to patients, carers and health professionals concerning a specific disease/condition.
The specialist nurse will:
* Work as part of the multidisciplinary team under the direction of line manager/Matron.
* Continue to develop advanced knowledge and skills within the specialist sphere of practice.
* Support with the planning, delivery and evaluation of care pathways for a designated group of patients under the direction of the line manager/Matron.
* Support the development of nursing practice within the specialist field through the use of evidence-based practice and in line with agreed national standards, and contribute to the body of nursing knowledge.
* Work as an autonomous practitioner within their sphere of practice.
* Continuously improve clinical standards in line with the clinical governance framework.
* Exercise accountability as set out in the NMC Code.
* Contribute to training and education within the specialist area.
* Provide specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within the specialist area.
* Provide direct access for patients, either face to face or telephone consultation.
* Represent the service nationwide in the absence of the line manager/Matron.
* Play a key role in ensuring that national and local standards/targets for the specialist area are met.
* Manage and lead nurse-led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions.
* Support and undertake research relevant to the needs of the specialist practice.

Skills and Knowledge
* Basic IT skills.
* Understanding of contemporary nursing issues.

Qualifications
* Degree in nursing studies or relevant clinical degree or evidence that currently working towards this (UK equivalent).
* Mentorship certificate/Practice Assessor training or demonstrable equivalent experience.
* Clinical skills accreditation or willingness to complete.
* Leadership and Management Course.
* Basic knowledge of financial management.
* Non-medical prescribing or willingness to complete.
