An exciting opportunity has arisen to join the Suffolk County Council Leaving Care Team as a Young Persons Advisor, working from Constantine House, Ipswich and Countywide.
Do you have experience of working with Care Leavers? Would you like to work in an incredibly varied, challenging and rewarding role? Are you passionate about making a positive difference to the lives of care leavers?
This is a fantastic opportunity to work within the Leaving Care Service and support care leavers from 18 up to 25, should care leavers wish to remain open to the service.
Your role and responsibilities
* Undertake regular statutory visits and chair Pathway Review meetings to manage and assess risk or any unmet needs, relating to housing, education/employment, finances, health, and emotional wellbeing.
* Provide advice and guidance on all these subjects to help your clients form secure foundations in their local community and increase their chances of living independently.
* Your primary role will involve working with over 18-year-old care leavers who will need ongoing support with housing, training and/or employment, and emotional and mental wellbeing.
* You'll hold case responsibility for these young adults and carry out Pathway Plans until at least their 21st birthday.
You will receive regular supervision from a manager/senior social worker and work collaboratively with team members, regularly attend team meetings, and be offered shadowing opportunities to broaden your understanding of care leaver protocols.
You will need
* Suitable practitioner level qualification at Level 3 or equivalent experience in a relevant area of work.
* Excellent communication skills with a commitment to listening to young adults around challenging topics such as family loss, mental health difficulties, substance misuse, and criminality.
* A thorough understanding of safeguarding concerns and specific risks associated with adult care leavers.
* Good organisational skills and competency with computer systems, paperwork, and case recordings.

We are a well-established Leaving Care Team. Our mission is to help our care leavers to develop their independence and make a successful transition into adulthood.
You will work with a specialist team of Social Workers and Personal Advisers to deliver high-quality social care services to young people leaving care throughout Suffolk.
