Other responsibilities: Referrals to LADO and Local Authority in line with safeguarding and assessments Educating Hospital staff about relevant safeguarding matters. Completing a full annual social care reassessment within the CPA format. Facilitating access to welfare rights, housing, advocacy services and associated support services Participating in 1:1 supervision With the service user’s permission initiate visits and maintain contact with patients families, carers and/or significant others offering support, information and where relevant to the care plan joint family and patient sessions which are enabling. To take a leading role in promoting anti-discriminatory practice in a way that links the service user individual circumstances to the broader, social, political, legal and cultural context (with relevance to care plan). To liaise with Social Services Departments to ensure that reports and attendance by a representative are available for Mental Health Review Tribunals.Where necessary, prepare reports and attend hearings and collaboration with Social Services Departments. To play a co-ordinating role in the process of Section 117 Mental Health Act 83 aftercare plans with the relevant community agencies which meet the needs of the service and takes full account of risk factors. In conjunction with the responsible Social Services Department or integrated NHS assist in the assessment of suitable after care placement working across professional boundaries. To take a lead role in regarding children visiting service users at the Brighton & Hove Clinic and liaison with children and families and child protection agencies operating within national guidelines. To maintain accurate and up to date records of their social work inputs to the care plan including Section 117 care planning and transfer summaries, which reflect the service user’s views. Ensure that service users have access to independent advice on financial legal and advocacy matters. To take the lead on safeguarding referrals and the subsequent liaison with the Local Authority safeguarding team and LADO To attend relevant Multi-Disciplinary Team meetings as required.
