Job Title: Director of Nursing Services
Reports to: Administrator
Job Objectives:
The primary purpose of this position is to plan, organize, develop and direct the overall operation of the Nursing Department in accordance with current applicable federal, state, and local standards, guidelines and regulations, and as may be directed by the administrator, to assure the highest degree of quality resident care can be maintained at all times.
Qualifications:
1. Graduate of Accredited School of Nursing, and currently licensed as an RN in the State of Wisconsin.
2. Experience as a supervisor in a hospital, long term care facility or other related health care facility; training experience in rehabilitative and restorative nursing practices is beneficial.
3. Understanding of what it takes to direct and supervise the care of the aged and chronically ill.
4. Read, write, and speak the English language fluently and communicate orally and/or in writing with residents, co-workers, families and public.
Essential Functions of Director of Nursing:
1. Independent decision making when circumstances warrant such action.
2. Tactfully manage and interact with personnel, residents, family members, visitors, government agencies/personnel and the general public.
3. Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to nursing home operation.
4. Possess leadership ability and the willingness to work harmoniously with and supervise professional and non-professional personnel.
5. Plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, procedures, etc., of the nursing service department.
6. Maintain the care and use of supplies, equipment, etc., and the appearance of nursing areas; regular inspection of nursing units for sanitation; order, safety and proper performance of assigned duties.
7. Seek out new methods and principles, and be willing to incorporate them into existing nursing practices.
8. Demonstrate patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning.
9. Use your senses of sight, hearing, and feeling at a level which allows you to hear, and interpret correctly, spoken directions, warning alarms and requests for information or guidance.
10. Lifting and caring equipment and supplies weighing 25 lbs.
11. Awareness of the exposure to infectious waste, diseases and blood borne pathogen exposure risk.
12. Professional demeanor in stressful situations when working with residents, personnel, administration, and physicians.
13. Willing to work beyond normal working hours and on weekends and holidays when necessary.
14. Commitment to be on call 24 hours a day, 7 days per week; allowing for arrangements to have been made with designee to accept said responsibility upon his/her absence.
15. Community, civic health, project involvement as appropriate.
16. Attend and participate in continuing educational programs designed to keep on abreast of changes in the profession, as well as to maintain your license on a current status.
Administrative Functions:
1. Plan, develop, organize, implement, evaluate, and direct the Nursing Services Department, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
2. Develop, maintain, and periodically update written policies and procedures that govern the day-to-day functions of the nursing service department.
3. Develop, maintain, and periodically update the Nursing Departments Policy and Procedure Manuals.
4. Develop methods for coordination of nursing service with other resident services to ensure the continuity of the residents’ total regimen of care.
5. Develop, maintain, and periodically update written job descriptions for each level of nursing personnel.
6. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
7. Maintain a reference library of written nursing material (i.e. PDRs, Drug Handbook, Federal and State Regulations, etc.,) that will assist the nursing service in meeting the day-to-day needs of the residents.
8. Develop, implement, and maintain an ongoing quality assurance program for the nursing department.
9. Assist the Administrator and Social Worker in developing, implementing and periodically updating the written procedures for the Discharge Planning Program.
10. Participate in survey (inspections) made by authorized government agencies and fill out all necessary forms that are requested.
11. Develop and implement a nursing service organization structure.
12. Make written and oral reports/recommendations to the Administrator as necessary/required concerning the operation of the Nursing Department.
Committee Functions:
1. Responsible for direction of Wound Care Program, Pain Management Program, and Infection Control Program.
2. Serve on, participate in, and attend various committees of the facility. (i.e., Infection Control, Pharmaceutical, Quality Assurance, Department Head, etc.,) as required and as appointed by the Administrator.
3. Attend and participate in developing the agenda for Care Plan Committee meeting as necessary.
4. Evaluate and implement recommendations from established committees as they may pertain to nursing services.
5. Oversight responsibility shared with MDS Coordinator for MDS completion and RUG utilization.
Personnel Functions:
1. Determine the staffing needs of the nursing department necessary to meet the total nursing needs of the residents.
2. Recommend the number and level of nursing personnel to be employed.
3. Assist the Administrator and Human Resource Coordinator in the recruitment and selection of nursing personnel.
4. Work with the Scheduler to assign a sufficient number of professional nurses for each tour of duty to ensure that quality care is maintained.
5. Work with the Scheduler to assign a definite number of CNA’s for each tour of duty to ensure the routine nursing care is provided to meet the daily nursing care needs of each resident.
6. Develop work assignments and schedule duty hours, and/or assist nursing supervisory staff in completing and performing such tasks.
7. Delegate to nursing supervisory personnel the administrative authority, responsibility, and accountability necessary to perform their assigned duties.
8. Ensure that professional nursing personnel have valid and current licenses and keep the written forms updated to show this as required by this State.
9. Make daily rounds of the nursing department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
10. Monitor absenteeism with the assistance of the Scheduler to ensure that an adequate number of nursing care personnel are on duty at all times.
11. Review and follow up on complaints or grievances made or filed by staff or residents, according to applicable federal, state, county, and facility regulations, policies, and procedures of Lafayette Manor.
12. Ensure that disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, sexual orientation, religion, handicap or marital status.
Nursing Care Functions:
1. Participate in the interviewing and selection of residents for admission into the facility.
2. Provide the Administrator with information relative to the nursing needs of the residents and the nursing department’s ability to meet those needs.
3. Inform all other departments of the new admission(s): expected time of arrival; room assignment; specialized equipment; etc.
4. Review nurses notes to ensure that they are informative and descriptive of the nursing care being provided, and that they reflect the residents’ response to the care.
5. Make rounds with physicians as necessary.
6. Encourage attending physicians to record and sign progress notes, physicians’ orders, etc., on a timely basis and in accordance with current regulations.
7. Schedule daily rounds to observe residents; to determine if nursing needs are being met.
8. Monitor medication passes and treatment schedules to assure that medications are being administered as ordered and that treatments are provided as scheduled.
9. Provide direct nursing care as necessary.
Staff Development Functions:
1. Develop and participate in the planning, conduction, and scheduling of timely in-service training classes that provide instruction “how to do the job”, and ensures a well-educated nursing department.
2. Develop, implement, and maintain an effective orientation program with the assistance of the Human Resource Coordinator; that orients the new employee to the facility, its’ policies and procedures, and his/her job position and duties.
3. Provide leadership training that includes the administrative and supervisory principles essential for Charge Nurses.
4. Encourage the nursing supervisory staff to attend and participate in outside training programs. Schedule times as appropriate.
5. Assist support services in developing, implementing, and conducting in-service training programs that relate to the nursing department.
Safety and Sanitation Functions:
1. Monitor nursing personnel to assure that they are following established safety regulations in the use of equipment and supplies.
2. Ensure that nursing work areas are maintained in a clean and sanitary manner.
3. Ensure that all residents’ rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.
4. Ensure that all nursing personnel receive instruction and follow established departmental policies and procedures.
5. Ensure that nursing personnel receive instruction and/or participate in all fire safety and disaster preparedness drills in a safe and professional manner.
6. Assist the Infection Control Committee in the development, implementation, and revising of written Infection Control Policy and Procedure.
7. Ensure that nursing personnel received instruction and follow established Infection Control Procedures and Standard Precautions.
8. Develop, implement, and maintain a procedure for reporting hazardous conditions or equipment.
Equipment and Supply Functions:
1. Recommend to the Administrator the equipment and supply needs of the department.
2. Monitor stock of medications, medical supplies, equipment, etc., to ensure adequate levels are maintained on the premises at all times to adequately meet the needs of the residents.
3. Develop and implement procedures for the safe operation of all nursing equipment.
4. Instruction all necessary personnel as to the proper and safe operation of all nursing equipment.
5. Develop, implement, and maintain procedures to ensure nursing supplies are used in an efficient manner to avoid waste.
Care Plan and Assessment Functions:
1. Assist as needed in preliminary and comprehensive assessment of the nursing needs of each resident.
2. Assist as needed in a written plan of care for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
3. Ensure that all nursing personnel follow the care plan, and that the care plan is updated as needed.
4. Oversee the review and revision of care plans and assessments as necessary, but at least quarterly.
5. Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.
Budget and Planning Functions:
1. Prepare and plan the Nursing Department’s budget and submit to the Administrator for his/her review, recommendation, and/or approval.
2. Keep abreast of economic condition/situations, and recommend to the Administrator adjustments in nursing services that ensure the continued ability to provide quality care.
3. Recommend to the Administrator the equipment needs of the nursing department.
Residents’ Rights Functions:
1. Maintain the confidentiality of all resident care information, observing all HIPAA, federal, state, county, and facility regulations, policies and procedures.
2. Monitor nursing care to assure that all residents are treated fairly, and with kindness, dignity, and respect.
3. Ensure that all nursing care is provided in privacy, and that nursing personnel knock before entering the residents’ room.
4. Ensure that all nursing personnel are knowledgeable of the residents’ rights.
5. Review complaints and grievances made by the resident and make a report to the Administrator promptly.
