The journey towards end of lifecan be a complex one. At Mary Ann we prideourselves on delivering the highest standard of care and support to patientsand their families, and constantly strive to ensure we reach as many people aspossible, whenever and wherever they may need us. The hospice serves a populationof approximately 190,000 people covering Nuneaton, Bedworth, Atherstone, Coleshilland the surrounding villages in northern Warwickshire.
The post holder will hold a current driving licence and have access to their own vehicle for duties associated with this role.
Main duties of the job
The post holder will hold a NVQ Level 3 in Health and Social Care or equivalent. They willbe able to demonstrate a commitment to continuing professional development anda willingness to complete further studies.
There is a preference that the post holder has experience in working in a palliative and end of life care however this is not essential. It is essential that the candidate has a passionate about improving and providing high quality palliative and end of life care in the community.
The post holder will possessstrong communication skills to enable them to build effective working relationshipswith their patients, carers and other healthcare professions at all levelsacross the healthcare sector. The post holder must be committed to providing personalised care by ensuring patients and their families are involved indecision making in relation to their health care.
It is essential that the post holder is confident and competent with IT skills as we use EMIS, which is an electronic patient records, training on EMIS will be provided.
The post holder will be ableto work independently as well as working as a member of Mary Ann's community team.

Job responsibilities
TheSenior Care Support Worker (SCSW) will be working within our community serviceswhich consist of the Hospice at Home (HAH) team, the Wellbeing programme andRapid Response (RR) which is a shared service with South Warwickshire NHSFoundation Trust. The role will bepredominately working within HAH and the Wellbeing programme working 7 days aweek between the hours of to At the current time we are piloting a change in hours working from 8am to 8pm. At the end of September this will be reviewed and hours may become permanent. There may be the occasionalrequirement to work within the Rapid Response team which will be internalrotation onto night shift.
Whenworking within the HAH team the SCSW will provide high quality palliative andend of life care to patients and their families within their own home. You willwork closely with our community and hospital colleagues from the NHS, socialservices, local care homes and other voluntary providers to support thosepatients who preferred place of death is to die at home.
Whenworking within the Wellbeing program the SCSW will work as part of a team tosupport patients, families and carers within the hospice setting to improvetheir quality of life, encouraging reablement and well-being models of care.
Onoccasions when required the SCSW will work with the Rapid Response team whichcovers a 24-hour service working alongside an experienced qualified palliativecare nurse to support them to provide urgent care for those patients who haveuncontrolled symptoms which require immediate intervention.

Essential
11. Level 3 Diploma/NVQ Level 3 in Health and Social Care for adults or equivalent
Desirable
12. Level 3 Diploma or equivalent certificate in palliative/end of life care
13. Level 2 or 3 Diploma/NVQ Level 2 or 3 in awareness of dementia
Experience
Essential
14. A minimum of 1 years care experience with palliative/end of life patients (any setting)
15. Able to work autonomously and as part of team
16. Evidence of excellent communication skills
17. Experience of supervising/supporting team members
Desirable
18. Experience of CQC compliance and evidencing regulations in practice
