This is an ideal opportunity for an ambitious and committed ANP/ ACP or Advanced Nurse Practitioner. It’s a chance to simplify and speed up the way patients receive care – by assessing them, planning their care, administering treatment and discharging or referring them to other healthcare professionals or external agencies, via primary care centres. You will also be using our electronic prescribing system.
Based in our various Out Of Hours (OOH) services, you’ll be working autonomously within your sphere of competency as part of a friendly and close-knit multi-skilled team that includes GPs, ACPs, ANPs, UCPs, mental health and dental nurses, prescribing pharmacists and pharmacy advisors.
What you'll be doing
* Facilitate clear patient pathways through primary, community, secondary and tertiary care.
* Take a person centred approach to service delivery, based on clear understanding and experience of the modes for service delivery.
* Be responsible and accountable for assessing health and/or social needs of patients within the PCC setting, using agreed clinical protocols and clinical guidelines software.
* Be able to incorporate the nurse’s role in health education within the unscheduled care environment.
* Maintain and develop clinical competence and knowledge of care delivery within PCC setting using both formal and informal training methods.
* Work closely with the medical practitioners in the provision of quality, evidence-based care to patients within the PCC setting or in patients' own homes.
What we’ll look for in you
What you will need:
* Recent experience as an autonomous advanced nurse/clinical practitioner in a primary/urgent care environment, with a proven ability to see, treat and discharge patients of all ages with unscheduled presentations.
* An ACP/ANP qualification, independent prescribing qualification and NMC/HCPC/RPharm registration.
* Excellent listening and communication skills, with good IT and keyboard skills.
