Continuing Healthcare (CHC) Nurse Coordinator
Band 6 (agenda for change). Please note we have two vacancies available.
Based in Kirkham House, Worcester or St Owen's, Hereford.

1. Registered Nurse - Adults
2. Registered Mental Health Nurses
3. Registered Learning Disability Nurses
We are looking for enthusiastic, self-motivated adult, mental health and learning disability registered nurses to join our Continuing Healthcare team. The role involves the coordination and chairing of CHC assessment meetings, package of care reviews and case management in relation to NHS commissioned care.
Prior knowledge of CHC would be beneficial but is not essential, as you will work within an experienced and supportive team who will support you to develop the relevant knowledge required.
Essential criteria for the role:
4. Registered nurse with the NMC
5. 3 years post registration experience
6. Driving licence and access to a car
7. IT literate
8. High standard of English literacy skills, written and verbal
This is a community-based job, the post holder will need to attend onsite assessments across Herefordshire and/or Worcestershire.
The successful candidate should be able to work independently to deadlines, have excellent communication skills to engage patients and/or their representatives. It is essential that the successful candidate has a very flexible approach, a positive attitude and is delivery focused.
Main duties of the job

9. To complete a comprehensive assessment to determine eligibility for NHS Continuing Healthcare.
10. To provide a case management service including reviews for eligible clients for an appropriate package of care or placement.
We are a friendly and welcoming team. You will be allocated a mentor and will be fully supported through your induction using a novice to expert programme. By the end of your first year you will achieved the following key skills;
11. Chairing complex MDT meetings
12. Clinical analysis and detailed report writing skills
13. In-depth knowledge of the NHS Continuing Healthcare National Framework and key supporting legalisation such as Mental Capacity Act
14. An understanding of integrated commissioning

Successful applicants will need to be able to work independently to deadlines, have excellent communication skills to engage patients and/or their representatives and be able to produce large, detailed written reports. You will therefore, need to be proficient in the use of Microsoft Word. There is also a continued need to complete remote assessments via Microsoft Teams, confidence with IT systems and leading meetings both face to face and remote is essential.
You will need to be a car driver as assessments can take place in clients homes, community hospitals and care homes across Herefordshire or Worcestershire and out of area where required. There is an opportunity to work flexibly to meet the needs of the service and this includes some working from home.
This a Monday Friday service, across Worcestershire and Herefordshire, based in either Worcester or Hereford. We have offices in Worcester & Herefordshire and promote hybrid working across the office and working from home.

15. First level qualification RGN, RMN or RNLD Registration
16. Relevant degree or extensive knowledge, skills and experience to equivalent level
17. Evidence of continuing Professional development
18. Significant post registration experience

19. Knowledge and understanding of current national and local policy / framework guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care
20. Knowledge and experience of working with safeguarding issues and policy guidance
21. Experience of working with patients with multiple/ complex needs
22. Ability to address complex or sensitive issues in an organised and appropriate manner both verbally and in writing
