We have an exciting opportunity for a 30 Hour Band 5 Community Staff Nurse to join the Single Point of Access for the District Nursing service.
The postholder will be required to provide a high standard of service providing virtual triage of all referrals into the District Nursing Service, therefore knowledge and experience of working within the District Nursing service is required.
The member of staff will be required to function as a member of the multi-disciplinary team, deputising in the Band 6 absence as required. They will undertake robust virtual triage on individual patient's care needs whilst providing support to individuals and their carers/families, escalating to the Band 6 and 7 as guided/required.
The role will include the supervision and instruction of unqualified members of nursing staff.
The members of staff would be required to attend training to maintain mandatory skills alongside developing new skills to meet the provision of care required.
The post holder will need a flexible approach and be adaptable to changing service needs.
The ability to speak Welsh is desirable for this role, as is the ability to travel between sites in a timely manner.
Main duties of the job
1. Provide and be accountable for the provision of a high standard of cost effective, safe, individualised nursing care to patients in the community setting through the assessing, planning, implementing and evaluating of patient care. This will be based upon evidence-based practice in line with legal requirements and health board policies relating to practice and the NMC Code.
2. Deputise for the Band 6 nurse in their absence and support them in the efficient running of the caseload.
3. Contribute to the induction and training of new staff members, student nurses, pre and post basic students.
4. Be flexible, adaptable in order to accommodate the needs of the service, be able to contribute to the performance of the community nursing team and develop professionally within the role.
5. Work alone and with confidence to make decisions without supervision. Work in line with the Health board Lone Worker policy.
6. Delegated responsibility for accepting appropriate referrals onto District Nursing Caseload.

Qualifications + Knowledge
* Level 1 Registered Nurse (Adult)
* Current NMC Registration
* Evidence of personal continuing development, statutory and mandatory training
* Awareness of Health Board policies, procedures and guidelines
* Ability to relate theory to practice
* Understanding of primary health care working
* Postregistration experience, in acute services.
* Teaching and assessing
* Understanding of clinical governance, health & safety, risk management awareness
* Community experience
* Willing to undertake further training as necessary.
* Evidence of post-registration education - Diploma / Degree Level
Experience
