An exciting opportunity has arisen to lead the nursing team of our Alcohol Care Team at Musgrove Park Hospital, you would join a small team with 2 other Specialist Nurses to help provide a specialised service to patients, providing a support service across the whole site. This team sits within a wider MDT team, also working closely with Hepatology and PLT sitting under the umbrella of Gastroenterology. The service we currently provide is relatively new and we are embracing how this is adapting and developing over time. Main duties of the job The post holder will be responsible for the day to day running of the Alcohol Care Team service. To provide specialist nursing advice and support to patients, their families and carers from the time of diagnosis and throughout the treatment and follow up phases of care. Assess all patients referred from the Emergency Department and inpatient wards and clinics and provide expert advice and signposting to community substance misuse service. Provide advice to all wards and departments regarding monitoring and management of alcohol withdrawal during patient admissions. To communicate and negotiate in complex situations and make clear, autonomous, and clinically safe decisions related to patient care. Provide a screening and brief intervention service to patients who have been identified as requiring intervention for drinking at a level that is assessed as harmful to their health. To actively seek patients within the acute wards to facilitate early intervention with a specific focus to admission areas including ED, the acute medical unit, and the surgical admissions unit. To liaise appropriately with all key members of the multi-disciplinary team including colleagues external to the Trust, specialist services, national networks, and users of the alcohol liaison service. In addition, the post holder will be required to liaise with colleagues across the Trust, divisional lead nurses, the divisional management team and appropriate corporate directorates and teams.
