To reduce the risk of loneliness and social isolation of people living with Dementia and their carers by providing meaningful person-centred interaction and support in peoples own homes and in the community. Support is based on individuals outcomes in line with agreed policies and procedures of Age UK Doncaster, in a manner which at all times promotes the privacy, dignity and independence of service users and their carers. Duties: Cultivate professional, trusted relationships with service users and their carers. Develop meaningful social interaction with service users by engaging in activities based on individuals outcomes, preferences, personal interests and hobbies. This may include outings or attending local activities and groups. Signposting to appropriate internal services / projects and external services / organisations. Feedback to line manager on achieved outcomes / concerns / incidents to line manager to ensure the appropriate level of support is in place. To work closely with the wider team to ensure appropriate cover at all times for all Services as per Age UK Doncaster Continuity Plan. Any other reasonable duties which may from time to time be required in line with the job purpose. Hours of work : Hours to be negotiated.
