Job summary This is an exciting opportunity within the Vascular Surgery directorate to work as an Advanced Clinical Practitioner (ACP). The Vascular ACP will be responsible for supporting vascular inpatients, reviewing patients via our admitting pathways and carrying out ACP-led clinics in the community and at spoke hospitals. The ACP will work closely with Vascular nurse practitioners and the Vascular Head of service to develop the service and ensure a robust pathway of care for our patients. Main duties of the job Clerking patients via our admitting pathways including ED, Surgical triage unit and inpatient wards. Preparing entries for patients in the Vascular Virtual Ward, attending the consultant-led Virtual Ward round three times per week. Co-ordinating care for patients on the virtual ward by linking closely with the community and hospital VW team. Capturing data pertaining to triage areasand Virtual ward in conjunction with coding team/analysts to demonstrate the value of these services to the wider department and to the Trust. Development of specific outpatient work streams for trainee and qualified ACPs with consultant oversight. Innovation and expansion of community services and Virtual Ward pathways in close conjunction with the Vascular Head of service. Responsibilities will include specialist in-reach for patients referred to Vascular services across the Trust. Key deliverables - Attendance at thrice weekly virtual ward meeting Daily AM presence on the ward round. Development of ACP role to incorporate semi-elective work e.g. Ward Discharge clinic.
