Further to new investment in the Pharmacy Service at Robert Jones and Agnes Hunt Orthopaedic Hospital we are looking for an experienced Pharmacist to lead the Pharmacy Homecare Service and be the Pharmacy lead for governance of high-cost drugs. The Trust has an excellent reputation for delivering Rheumatology and Metabolic Bone Services to the whole of Shropshire and neighbouring counties in England and Wales and the Pharmacy Homecare Service supports these patients in delivering cutting edge medicines to over 2,300 patients. This demand is growing as new therapies and NICE guidance become available. As the Senior Pharmacist for Pharmacy Homecare, you will work closely alongside a team of experienced technical, professional and homecare administration staff to maintain and develop a high standard of Medicines Homecare to patients with often long term and complex conditions who receive their medication at home. The role will involve direct patient communication, liaison with clinicians, clinical pharmacists and nurse specialists as well as external homecare and pharmaceutical companies. In addition you will be supported to develop your clinical skills in rheumatology/metabolic bone health by participating in regular clinics where you will be encouraged to develop your prescribing skills.

Main duties of the job
We are looking for a pharmacist who has significant experience of working in primary or secondary care, ideally with a knowledge and understanding of the biological medicines used in the management of rheumatological conditions. You will need excellent organisational skills and the desire to provide a high quality, safe, effective and efficient service to our patients. You must be able to work under pressure, prioritise work and be able to work alone as well as part of a team. You will also supervise and manage other members of the team. The role ensures that RJAH homecare services are of a consistently high quality and in line with the Royal Pharmaceutical Society Homecare Standards and local policy and procedures.
The role will work closely with the Trust Finance Team and our partners across the ICS to ensure that medicines are appropriately invoiced to the correct commissioners and the Trust adheres to the Best Value Medicines principles. The postholder will lead on establishing reporting mechanisms to ensure good financial governance and oversight.
The role also includes a regular operational commitment to maintain a working knowledge of the Trust processes. You'll need to be able to work over 7 days of the week, hours of work as required. The department offers a weekend (Saturday) and bank holiday service which you will be expected to participate in. Pharmacists do not currently provide an on-call service.

Job responsibilities
1. Liaise and communicate effectively with all members of the homecare team involved in the prescribing, supply and delivery of Homecare medicines including Homecare providers.
2. Input, search and retrieve data relating to the Homecare service using Trust computer systems and databases.
3. Provide advice and operational support to patients, carers, homecare providers and Trust staff regarding homecare enquiries and refer to the relevant Lead Technician Medicines Homecare or specialist pharmacists if necessary. This may include communicating with patients who are distressed, frustrated and those with disabilities and being sensitive to their needs.
4. Ensure that the correct orders are raised, released, delivered, and issued via the Pharmacy computer system.
5. Communicate with ICSs / LHBs regarding funding issues for Homecare patients.
6. The development and maintenance of Standard Operating Procedures (SOPs).
7. To assist in the monitoring and audit of homecare related incidents, ensuring where appropriate they are reported on Datix. Liaise with homecare providers to investigate such issues according to the RoyalPharmaceutical Society Homecare standards.
8. Undertake research and development activities as directed, collating information, reports on homecare supply, delayed and incorrect invoicing

Essential
9. Master's degree in pharmacy or equivalent
10. Postgraduate Diploma in Clinical Pharmacy or equivalent demonstrable experience
11. Registered as a pharmacist with The General Pharmaceutical Council
12. Registered Independent Prescriber or prepared to undertake the qualification (full support will be provided)

13. MSc in Clinical or Technical Pharmacy

16. Extensive post-registration experience with substantial clinical experience
17. Extensive experience providing a pharmacy clinical service to patients

21. Evidence of on-going revalidation with the GPhC
