Job Overview
Northamptonshire Healthcare Foundation Trust NHS is delighted to offer the opportunity for a Senior Physiotherapist to join the Beechwood Rehabilitation Unit team, as part of the Community Beds service within Northamptonshire. The post holder will provide assessment, set goals, and rehabilitation to our adult and older adult inpatients with physical rehabilitation needs, plus work with the MDT to establish safe discharge plans.
Main Duties of the Job
1. Complete initial and ongoing patient assessments, support patients to identify SMART goals, utilize clinical outcome measures, and develop, deliver, and progress bespoke treatment plans.
2. Work collaboratively within the team and with external providers to:
* Enable patients to work towards goals relevant to the in-patient community hospital setting.
* Support the patient and family members/carers regarding discharge planning.
* Identify further rehabilitation needs, including onward referral to other services as appropriate.
* Support in developing the service in line with national and local strategies.
* Demonstrate critical thinking in the clinical decision-making process and evaluate the effectiveness of treatment through autonomous and accountable decisions in Physiotherapy practice supported by current research and in accordance with NHFT protocols and policies.
* Communicate condition-related information, sensitive diagnostic & prognostic information to patients/clients, carers & relatives, requiring empathy & reassurance.
1. Provide leadership, education, and support to other team members through the promotion of innovative strategies to manage change and improve service delivery.
Minimum Requirements
HCPC registration and relevant degree/qualification in Physiotherapy.
Good communication and organisational skills.
