Work as a triage staff nurse with a professional and clinical knowledge of community nursing. Whilst not providing regular face-to-face patient contact, the post holder will need to maintain an up-to-date knowledge of the clinical aspects of community nursing in order to resolve clinical queries over the phone and to enable effective remote assessment and clinical triage. This includes palliative care (particularly symptom management and support to families in the last few days of life), wound care, medication management, diabetes management, catheter, and bowel care.
Main duties of the job
1. Demonstrate competence in remote clinical triage through successful completion of induction, probation, and through performance review at monthly supervision with line manager.
2. Demonstrate MPFT Values at all times.
3. Have an understanding of the scope of the community nursing service and the role of other MPFT and external services within each Primary Care Network to ensure referrals are directed to the most appropriate service (including directing to emergency services where needed).
4. Occasional delivery of care to patients in a community setting in order to maintain up-to-date clinical experience.
