Job summary
The Humbleyard Practice has a fantastic opportunity for an experienced enhanced clinical practitioner (ECP), who will play a key role in providing flexible and responsive care both within the practice and through home visits. This role will primarily be based in the community, with designated clinic time within The Humbleyard Practice. Working hours are flexible but will include a structured home visiting rota in the afternoons.
You will be responsible for assessing, diagnosing, treating, and managing patients across a wide range of acute and long-term conditions.
The successful candidate will demonstrate flexibility in attitude and behaviour. They will be keen to undertake new ways of working and delivering healthcare, committing to under-taking the professional development required to adapt to changing circumstances.
Main duties of the job
* Conduct comprehensive home visits for housebound patients, including those requiring urgent assessment, chronic disease management, end-of-life care, and post-hospital discharge reviews.
* Work within an established home visiting protocol to determine the urgency and type of response needed for each patient.
* Provide clinical assessment, diagnosis, treatment, and prescribing (within scope of practice) for a range of conditions.
* Monitor and manage patients with long-term conditions, ensuring care plans are in place and reviewed regularly.
* Work closely with GPs, community nursing teams, social care, and other relevant services to provide coordinated care.
* Identify safeguarding concerns and escalate appropriately in line with practice policies.
* Maintain accurate and timely records of all patient encounters in the clinical system.
* Support urgent care provision, triaging and managing on-the-day demand when required.
* Provide independent consultations for minor ailments, long-term condition reviews, and preventative health measures.
* Support medication reviews and deprescribing initiatives for patients with complex needs.
* Assist in the management of long-term conditions.
* Participate in clinical meetings, governance reviews, and quality improvement initiatives.
* Act as a clinical resource for other team members, supporting learning and professional development.
* Support the integration of new pathways and initiatives to enhance patient care and experience.

Qualifications
Essential
* A Masters Degree in Clinical Practice.
* Be a Registered General Nurse with experience of working within primary care.
* Learning and Assessing in Practice Qualification or equivalent practice assessors training.
* Independent prescriber (V300 qualification).
Experience
Essential
* Experience in primary care, community nursing, or urgent care settings.
* Strong clinical assessment, diagnostic, and decision-making skills.
* Experience in managing long-term conditions and frailty.
* Understanding of safeguarding procedures for adults and children.
* Full UK driving licence and access to a vehicle for home visits.
Desirable
* Experience in end-of-life care and palliative support.
* Knowledge of local community services and referral pathways.
* Previous experience in a home visiting role within primary care.
