What you will do:
1. Carry out Care Act assessments and strengths-based support planning, optimising available resources.
2. Conduct carers assessments and capacity assessments with subsequent Best Interest decision-making processes.
3. Engage in Continuing Health Care assessments and compelling interdisciplinary and multi-agency working collaborations.
4. Lead safeguarding investigations and support individuals transitioning into alternative accommodations.
5. Involve yourself in crucial resettlement processes, advocating for out-of-area placement returns to the local scene.
6. Prepare detailed reports including Ministry of Justice Tribunal reports and Social Circumstances reports for Mental Health Tribunals.
7. Equip yourself for vital Hospital Discharges and Community Treatment Reviews. Handle Court of Protection applications with precision.
Qualifications and Experience:
1. A qualified Social Worker with a proven track record working with adults with learning disabilities in a statutory role.
2. Preferred experience with liquid logic systems is advantageous.
