Job summary You will be working closely with GP practices, prioritising and responding to practice needs with home visiting requests included within the role. Home visits are primarily in patients homes, with treatment plans, referrals or admissions as agreed with the duty GPs as appropriate. Experience with EMIS is useful - otherwise full training will be given - although good IT skills are essential.
