Job summary Central London Community Healthcare (CLCH) NHS Trust is seeking a dynamic and compassionate Community Frailty Consultant to lead and deliver high-quality care across Brent and Harrow. This pivotal role provides clinical leadership for the Urgent Care Response Teams (Rapid Response), Enhanced Care Home Team (ECHT), Integrated Case Management (ICM), and Falls Services. You will be the clinically accountable clinician, working with multidisciplinary teams to prevent hospital admissions, manage complex cases, and deliver seamless, patient-centred care. You will be responsible for delivering excellent clinical outcomes, taking accountability for patient care, and providing guidance and leadership. To be considered for this role, you must be a fully registered member of the General Medical Council with a current license to practice. Main duties of the job As a Community Frailty Consultant, you'll be responsible for delivering excellent clinical outcomes, taking full accountability for patient care, and providing guidance and leadership to the Urgent Care Response team. This role also involves ensuring the integration of services across Brent and Harrow, two ethnically diverse boroughs with growing populations.
