Central London Community Healthcare (CLCH) NHS Trust is seeking a dynamic and compassionate Community Frailty Consultant to lead and deliver high-quality care across Brent and Harrow.
This pivotal role provides clinical leadership for the Urgent Care Response Teams (Rapid Response), Enhanced Care Home Team (ECHT), Integrated Case Management (ICM), and Falls Services. You will be the clinically accountable clinician, working with multidisciplinary teams to prevent hospital admissions, manage complex cases, and deliver seamless, patient-centred care.
You will be responsible for delivering excellent clinical outcomes, taking accountability for patient care, and providing guidance and leadership.
Your structured weekly schedule will balance clinical responsibilities and management duties, allowing for efficient time management while prioritising high-quality care.

To be considered for this role, you must be a fully registered member of the General Medical Council with a current license to practice. You should be on the Specialist Register for Geriatric Medicine or within six months of obtaining your Certificate of Completion of Training (CCT) at the time of interview.
Part time/ Job share working patterns will be considered.
Main duties of the job
As a Community Frailty Consultant, you'll be responsible for delivering excellent clinical outcomes, taking full accountability for patient care, and providing guidance and leadership to the Urgent Care Response team. This role also involves ensuring the integration of services across Brent and Harrow, two ethnically diverse boroughs with growing populations.

* MRCP (or equivalent) full registration with GMC

* Education/Qualification: Higher degree (MSc, MD, or PhD) or equivalent, Fellowship of the Royal College of Physicians.
Experience

* Experience in care of the elderly, and A&E/urgent care settings

* Experience in managing frailty, acute disease, and chronic conditions such as heart failure, COPD, dementia, and depression.
