Who You Are We seek a dedicated and HCPC-registered Occupational Therapist with experience in adult social care or community settings. You should possess strong case management skills, have experience in goal setting and strength-based interventions, and be proficient in conducting complex movement and handling assessments and care planning. IT proficiency with a preference for experience in the Mosaic system is desired. What the Job Involves This frontline role is based at the Westminster Community Independence Service (CIS) Reablement Team, where you will support residents in regaining independence following hospital discharge or health crises. Working Monday to Friday with an option for flexible hours, you'll deliver skilled Occupational Therapy interventions, manage cases with goal-focused reablement plans, conduct holistic assessments, and develop and implement intervention plans, including complex assessments and equipment allocation. Collaboration with multidisciplinary teams, including Occupational Therapists, Independent Living Assessors, and Social Workers, is key to ensuring effective service delivery. You'll maintain accurate case records using the Mosaic system and receive ongoing professional development and supportive supervision.
