An opportunity has arisen for a qualified Social Worker or Occupational Therapist to join our Home First Team (Hospital Assessment and Care Management Social Work) in Falkirk. We encourage applications from practitioners who have experience working in a busy and dynamic hospital discharge team working with adults across all Forth Valley hospital sites and intermediate care settings. You will have knowledge and experience of assessment and care planning in hospital discharges and statutory legislation and guidance relating to adult services social work, together with an ability to manage complex casework and to confidently assess and manage risk.
Due to service redesign, applications are welcomed from experienced qualified Social Workers and Occupational Therapists who meet our Advanced Practitioner criteria. This is based on a Social Worker/OT who is committed to practicing at a higher level of competence derived from the accumulation of experience, level of expertise (academic and developmental) and the ability to confidently engage with situations that present greater risk and complexity.

You should be qualified for a minimum of three years, have experience of chairing meetings and a proven track record of supporting team practice development in a number of ways, such as mentoring, facilitating student placements, groups or communities of practice.
