Job Description:
We are looking for a skilled and highly motivated Practice Lead to join the Exeter Community Mental Health teams.
The service is embarked on a challenging but exciting journey with the implementation of the Community Mental Health Framework. The successful applicant will be an innovative and compassionate clinical leader, who is motivated to support the teams to deliver individualised, person-centred care and will make a significant contribution to service development of the Exeter City Community Team. You will join a rich multi-disciplinary team which includes Nurses, Occupational Therapists, Social Workers, Psychologists, Psychiatrists, Peer Support Workers, Clinical Associate Psychologists and Community Support Workers. We are also forming closer working relationships with colleagues in Primary Care and the Voluntary Sector.
You will be an experienced mental health professional (Registered Nurse, Social Worker or Occupational Therapist) who has a genuine interest in collaborative working, patient safety and delivering high quality care.
Main duties of the job
The Community Practice Lead is responsible for ensuring compliance with quality & safety standards within their assigned clinical pathways / teams. They will ensure that all individuals receiving care and treatment from DPT services do so in line with Trust strategic objectives (safe, timely, personalised, sustainable and recovery focused) and are safeguarded from any harm.
Workforce planning, audit, compliance with CQC standards, serious untoward incidents, complaints and compliments and associated learning, and clinical practice education are the key responsibilities of this role. The post holder will ensure clinicians work within DPT policy and practice standards, and that they adhere to professional registration requirements.
The post holder will be an effective role model for clinical excellence across their assigned clinical pathway. They will demonstrate visible leadership and take responsibility for ensuring a therapeutic environment.

The post holder will be responsible for the development and monitoring of the assigned care pathway quality and improvement plans. The post holder, in conjunction with the CMHT management team, will ensure all clinical effectiveness systems are embedded and sustainable - this will include audit and evidence based care.
Participate in, and lead as required, the development, implementation and evaluation of strategies and clinical policies to ensure the service continues to evolve to meet the needs of the individuals and / or their carers.
Ensure that individuals care pathways are monitored and that individuals transition through the service appropriately.
Ensure that governance and assurance systems are in place for care pathways / clinical teams and provide expert professional advice with regard to clinical care, incidents and complaints.
Ensure that services are delivered by practitioners with the appropriate knowledge, skills and competencies.
In conjunction with the Community Team Managers, ensure that care is provided within the CMHT resources.
Take the lead, in consultation with the CMHT management team, in being responsible for:
The implementation of the Care Programme Approach within all Trust services is in accordance with the recovery co-ordination policy.
The coordination of multi-disciplinary team working and engagement in care pathway delivery in line with practice standards.
Ensuring the safety and effectiveness of the service through the implementation of effective risk management, governance and assurance systems.
Develop the LDU workforce development plan.
Line Management Responsibility
Be responsible for ensuring the clinical supervision and Continuous Professional Development of pathway staff.
Provide appropriate cross-cover for supervisions and PDRs as required.
Additional Responsibilities
All practice leads will hold a portfolio of LDU responsibilities these will include: ensuring the quality and triangulation of provider compliance assessment tools and responsibility for updating and monitoring risk registers.
They will also take responsibility for key practice development projects across the directorate as assigned by the Head of Practice and Professions.
All Community Practice Leaders will hold an LDU portfolio based on individual job planning e.g. safeguarding, complaints.
Person Specification
Knowledge
* An excellent understanding of the NSF and Mental Health Act/ Mental Health Capacity Act requirements.
* CPA Process
* Understanding of recovery principles in Mental Health
* Knowledge of infection control
* Working knowledge of safeguarding processes (adult and child)
* Knowledge and understanding of the Community Mental Health Framework
* Aware of community structures and social care options and processes

Qualifications
* A professional mental health qualification to degree level or equivalent
* Post graduate qualification relevant to specialism
* Training in specific therapeutic skills that facilitate the care pathway e.g. Non-medical prescribing, CBT, DBT
* Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff
Experience
* Extensive experience of managing a busy caseload of clients with a range of mental health problems.
* Substantial experience of working at a senior clinical level.
* Working within a community mental health setting in either the statutory or voluntary sector.
* Providing clinical supervision and managing staff.
* Experience of assessing and managing complex risk / safety for both individuals and teams
* Ability to assess and co-ordinate care that includes physical health presentations
* Resource management.
* Setting up or working within a new service.
